Bring a large pot of salted water to a boil. Cook the penne pasta according to package instructions until al dente. Drain and set aside.
Heat olive oil in a large skillet over medium-high heat. Add the ground Italian sausage, breaking it up with a spoon. Cook until browned and cooked through. Drain excess fat if needed.
Add the minced onion and garlic to the skillet. Sauté until the onion is translucent and fragrant.
Reduce heat to medium-low and pour in the heavy cream. Stir and let it simmer gently to combine flavors.
Stir in the baby spinach and cook until wilted, about 3 minutes.
Add the shredded parmesan cheese and stir until melted and the sauce is creamy.
